There is a $50 non-refundable administrative fee once you have registered for the course.
You must be an active QMA in the state of Indiana and have a facility to complete the validation of your skills with.
Submit your ID, SS card, QMA certificate, and CNA certificate prior to the first day of class. You will not be able to attend class without all of the required paperwork!
